Purpose/Background: The standard treatment for transverse colon cancers often involves an extended right hemicolectomy or subtotal colectomy. In keeping with oncologic principles and with a D3 node dissection, vessels must be ligated high in order to ensure an adequate lymph node yield. In transverse colon cancer, along with the blood supply to the right and left colon, it is advisable to ligate the middle colic vessels close to their origin, which poses the challenge of managing the short common trunk of these structures.

Methods/Interventions: The patient was a 32 year old male with transverse colon cancer in the context of Ulcerative Colitis Cancer. This video highlights the important steps of a laparoscopic subtotal colectomy with isolation and ligation of the middle colic vessels at their origins and a D3 node dissection. This technique can be applied to any procedure requiring ligation of the middle colic vessels.

Results/Outcomes: This video displays our stratified approach to the safe isolation of the root of the middle colic vessels. There were no intra-operative complications. The patient was discharged on POD 3. The final pathology revealed T3N2 tumor with 6/149 lymph nodes positive and positive nodes at the root of the middle colic vessels and one node along the gastroepiploic chain.

Conclusion/Discussion: Due to their variable anatomy and difficult location, identifying the origin of the middle colic vessels is not a simple task in laparoscopic surgery. Having a safe and methodical approach to high ligation of the origin of the middle colics may improve oncologic outcomes.
