Purpose/Background: Rectal prolapse is a common anorectal disease that seriously affects the quality of life of patients. In the treatment of severe rectal prolapse, a single surgical approach is often ineffective, and a combination of multiple surgical approaches is urgently needed. For elderly patients performing laparoscopic anterior rectal fixation surgery to remove the Douglas fossa to treat complete rectal prolapse is effective and safe, with a recurrence rate of only 3%, but with the passage of time the repair effect gradually declines, especially recurrent rectal prolapse is not effective. That is why surgical improvement is more and more important nowadays with continuous progress.

Methods/Interventions: During the operation, we first explored the lowest position of the peritoneal reflexion depression and the starting position of rectal prolapse after returning the bowel back to the abdominal cavity through the application of laparoscopy, and separated the presacral fascia by freeing the right mesentery of the rectum and the rectovaginal septum through the laparoscope. The rectum was suspended using a silk thread while the hypertrophied hernia wall was removed by ultrasonic knife to remove the Douglas fossa and elevate the rectum to consolidate the surgical result.

Results/Outcomes: For the treatment of third-degree rectal prolapse, the use of the laparoscopic rectal-suspended Chen's modified method increases the efficiency and decreases the recurrence rate of rectal prolapse.

Conclusion/Discussion: For the effective treatment of third-degree rectal prolapse, the application of the laparoscopic rectal-suspended Chen's modified method significantly increases the treatment efficiency and remarkably decreases the recurrence rate of rectal prolapse. This innovative approach not only offers a more targeted and precise treatment option but also brings greater hope and better therapeutic outcomes for patients suffering from this challenging condition
