Purpose/Background: This video demonstrates a case in which endoscopic clip placement with subsequent radiographic evaluation is used as an adjunct to tattooing for localization of polyps intra operatively.

Methods/Interventions: Two endoscopic clips were placed during a colonoscopy to mark the locations of previously biopsied but non localized transverse colon polyps. A subsequent abdominal X ray was taken which revealed the clips to be located in a redundant portion of the proximal transverse colon at the hepatic flexure. As intraoperatively the tattoo injected endoscopically was not seen, this radiographic representation of the polyp location allowed appropriate margins to be taken during intraoperative decision making.

Results/Outcomes: The specimen was opened and there was an appropriate margin between the distal most clip and the edge of the specimen. Both clips were seen clearly in place on review of the gross specimen.

Conclusion/Discussion: We propose that endoscopic clip placement with radiographic localization can be considered as an adjunct to tattoo injection in the case of patients with difficult anatomy to guide intraoperative decision making and allow for a safe margin to be obtained.
