Purpose/Background: Surgery remains the only definitive treatment for sacrococcygeal pilonidal sinus. Traditional surgical methods often involve the excision of large amounts of gluteal tissue, leading to prolonged healing times and various postoperative complications, which can significantly affect patients' postoperative quality of life. In recent years, minimally invasive surgery for sacrococcygeal pilonidal sinus has gained increasing attention.This study reports our hospital's experience with the use of laser ablation technology for the treatment of sacrococcygeal pilonidal sinus from March 2019 to July 2024.

Methods/Interventions: We conducted a retrospective analysis of 285 patients with sacrococcygeal pilonidal sinus who underwent laser ablation closure at our institution between March 2019 and July 2024. All surgeries were performed by the same surgeon following a standardized surgical procedure, utilizing 1470nm radial laser fiber for ablation. Patient data were collected through the electronic medical record system and telephone follow-ups, including preoperative information such as demographics, lesion characteristics, and prior surgical history, as well as postoperative clinical information such as complications (wound bleeding, wound infection), postoperative pain, sinus closure, and recurrence. Postoperative pain was assessed using the Visual Analogue Scale (VAS).

Results/Outcomes: A total of 285 patients were included, with 84.3% (n=241) being male and 15.7% (n=43) female. Of these, 33 patients had a history of prior pilonidal sinus surgery. The postoperative bleeding rate was 8.4% (n=24), with no reported cases of wound infection. Pain levels, as assessed by VAS, were as follows: 2 (1, 3) on the day of surgery, 2 (1, 3) on postoperative day 1, 1 (0, 2) on day 7, 0 (0, 1) on day 14, and 0 (0, 0) thereafter. The overall sinus tract closure rate was 95.4% (n=272), with a closure rate of 96% (n=242) in primary cases and 90.9% (n=30) in recurrent cases. The overall recurrence rate was 4.6% (n=13), with a recurrence rate of 4.0% (n=10) in primary cases and 9.0% (n=3) in recurrent cases.

Conclusion/Discussion: Laser ablation is a safe and effective minimally invasive treatment for sacrococcygeal pilonidal sinus. Compared to traditional surgical methods, laser ablation is associated with fewer complications, faster postoperative recovery, and high sinus tract closure rates with low recurrence rates. It offers satisfactory outcomes for both primary and recurrent cases, with better outcomes in primary cases than in recurrent ones.
