Purpose/Background: To evaluate the efficacy of FIPS surgery compared to traditional anal fistula incision surgery in wound healing after low position simple anal fistula surgery.

Methods/Interventions: In this experiment, 100 hospitalized patients with low simple anal fistula who underwent initial surgery were randomly divided into an experimental group of 50 cases and a control group of 50 cases. The experimental group underwent fistulotomy and primary sphincteroplasty (FIPS), while the control group underwent traditional anal fistula incision. The wound healing time, healing rate, and complications between the two groups were compared.

Results/Outcomes: Among the 100 patients admitted to this trial, the cure rate of the experimental group was 88%, the significant effective rate was 100%, and the effective rate was 100%. The cure rate of the control group was 72%, the significant effective rate was 96%, and the effective rate was 100%. There was a significant difference in the total time for wound healing (p=0.026 < 0.05). There was a difference in wound pain score 14 days after surgery (p=0.004 < 0.05); The difference in wound edema score was significant on the 7th day after surgery (p=0.013 < 0.05); The Jorge/Wexner anal function scale score showed a significant difference on the 28th day after surgery (p=0.007 < 0.05); Anorectal pressure measurement showed an increase in the maximum systolic pressure of the anal canal before and after treatment (p=0.000 < 0.050); There was a significant difference in resting pressure of the anal canal between the two groups before and after treatment (p=0.000 < 0.050).

Conclusion/Discussion: Compared with traditional anal fistula incision, FIPS surgery has the advantages of improving wound healing rate, shortening wound healing time, reducing postoperative wound edema, and having less impact on anal function in the postoperative wound healing of low simple anal fistula. Moreover, the therapeutic effect is significantly improved compared to traditional fistulotomy, with fewer complications.
