ACPGBI - Impact of Surgical Intervention on Patient-Reported Gastrointestinal Functional Recovery from Small Bowel Obstruction: Insights from the SnapSBO Cohort

Background: Small bowel obstruction (SBO) is a commonly encountered surgical emergency. The PRO-diGI patient-reported outcome measure (PROMs) tool assesses GI recovery across the domains of Appetite, Nausea, Bowel Function, and Wellbeing as well as a rating of overall GI health. This study explored how demographic and treatment factors influence GI PROMs during recovery.

Methods: SnapSBO is a prospective, international, cohort study of patients with SBO. Key management data were collected, and PROMs (PRO-diGI) were obtained during outpatient follow-up. We compared population demographics for those with and without PROMs data for equivalence. Regression models explored the association of demographic and treatment factors with PROM scores.

Results: Of the 1,734 participants, 978 had no PROMs data; at least one PROM domain was available for 756 patients. Both groups were similar, except the PROMs group included more operated patients (62% vs 41.8%, p < 0.001). Complete PROM data was present for 644 patients. Surgical intervention was linked to better Nausea (coefficient & (95% CI) 5.9 (1.1 to 11) and Overall scores (6.8 (0.54 to 13), while surgical complications negatively impacted Nausea (-9.3 (-17 to -1.7), Wellbeing (-17 (-29 to -4.3), and Overall scores (-12(-22 to -1.4)). Previous non-operative SBO episodes were associated with lower Appetite scores (-7.3 (-13, to -1.7).

Discussion: This study suggests that when SBO requires surgical intervention patients report improved functional recovery. Unanticipated consequences, such as reduced appetite following non-operative SBO resolution, merits inquiry as potential indicators of patients who may benefit from operative management to improve quality of life.
